Information for Creditors

 Charitable Gift Receipts for Creditors

Any creditor who does not wish to go through the trouble of filing a claim may elect instead to receive a tax deductible charitable gift receipt in the amount of your claim.

The charitable gift receipt will be provided to you upon submission of a signed written statement by you, preferably on your letterhead, including the following:

[CREDITOR NAME HERE], on this [DATE HERE] day of December, 2011, hereby fully discharges the debt owed to it by the North Central Florida YMCA. Inc. in the amount of [$AMOUNT HERE] as a charitable contribution in support of the North Central Florida YMCA, Inc.'s reorganization efforts.

If you have already filed a claim and prefer not to continue with the proceedings, you may withdraw your claim, provide proof that the claim was withdrawn along with a written statement as indicated above, and the North Central Florida YMCA, Inc. will also issue a charitable gift receipt to you.  For information and assistance regarding withdrawing your claim, contact Heather Sanders at 352-335-0400.
